Satellite Seminar Registration Form
Each attendee must submit a separate registration form providing the requested information and then clicking "Register".
Unless otherwise noted, each satellite seminar will be held at Old Dominion University's
Peninsula Center, 600 Butler Farm Rd., Suite 200 Hampton, Virginia 23666 Telephone: (757) 766-5200.  Click here for detailed directions.
Payment for seminar(s) should be made on-site, at the registration desk by cash or check.  The price of each seminar includes a booklet and lunch.  A reservation is a commitment to pay the full amount unless cancelled in writing 5 days prior to the seminar.  Cost:  $10.00 members; $20.00 non-members.
